Create custom appointment form using shortcode & get a full list of submitted form on admin panel. You can approve/cancel an appointment from plugin settings.

== Installation ==

Extract the zip file and just drop the contents in the wp-content/plugins/ directory of your WordPress installation and then activate the Plugin from Plugins page.

Que: Did this plugin support shortcode?

Ans: Yes. You have to put [afm_contact] to display appoint form on any page.

Que: Can I change form css styles?

Ans: Yes. You can add css using custom css editor from admin panel.

Que: Can I add, edit or remove form fields from plugin settings?

Ans: No. This feature is not available in current plugin version but we are working on it. Please keep continue with us & you can manage custom form fields in our next version 2.0.0.
