This extension allows you to easily add jQuery powered AJAX forms to any page with an unlimited number of fields. It's easy to style and has an uncluttered easy to use admin interface.

Form fields can be inputs, text areas, check boxes, radio buttons, select boxes and can be set as required. Extract the zip file and just drop the contents in the wp-content/plugins/ directory of your WordPress installation and then activate the Plugin from Plugins page.
